The basics

What is tirzepatide?

Tirzepatide is a dual GIP and GLP-1 receptor agonist. It acts like two natural gut hormones, glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1), that your body releases after you eat. Together they reduce appetite, help you feel full sooner, slow how fast your stomach empties and improve how your body handles blood sugar. Zepbound is approved for adults with obesity, or overweight with at least one weight-related condition, along with a reduced calorie diet and increased physical activity.

How it works

How tirzepatide works

Lower appetite

Tirzepatide acts on appetite centers in the brain, so hunger and food cravings drop.

Fuller, longer

It slows stomach emptying, so smaller meals feel satisfying.

Better blood sugar control

It helps the pancreas release insulin when blood sugar is high and improves insulin sensitivity.

Two hormones, not one

Adding GIP to GLP-1 appears to boost weight loss and may improve how the body handles fat.

Results

Tirzepatide weight loss results

In the SURMOUNT-1 trial, adults with obesity lost an average of about 15% of their body weight on 5 mg, 19.5% on 10 mg and 20.9% on 15 mg over 72 weeks, compared with about 3% on placebo. In SURMOUNT-5, a head-to-head trial, tirzepatide led to about 20% average weight loss compared with about 14% on semaglutide over 72 weeks.

Results vary. Your weight loss depends on your dose, how long you stay on it, your diet, whether you exercise regularly, sleep and mental health. Most people notice less hunger in the first weeks, and weight loss builds over months as the dose increases. Tirzepatide works best as part of a treatment plan with lifestyle changes for long term weight management.

Average weight loss at 72 weeks

  • 5 mg: about 15%
  • 10 mg: about 19.5%
  • 15 mg: about 20.9%
  • Placebo: about 3%

Candidates

Who is a good candidate for tirzepatide?

Tirzepatide for weight loss is FDA approved for adults with obesity, meaning a body mass index (BMI) of 30 or higher, or adults with a BMI of 27 or higher and weight related medical problems such as high blood pressure, high cholesterol, type 2 diabetes, prediabetes, sleep apnea or heart disease. Zepbound is also approved to treat moderate to severe obstructive sleep apnea in adults with obesity.

A history of pancreatitis, gallbladder disease, kidney disease or diabetic retinopathy needs a careful conversation first. Tia reviews your medical history and all prescription and nonprescription medications, including over the counter products and nutritional supplements, before prescribing.

Tirzepatide is not right for you if

  • You have a personal or family history of medullary thyroid carcinoma
  • You have multiple endocrine neoplasia syndrome type 2 (MEN 2)
  • You are pregnant
  • You have had a serious allergic reaction to tirzepatide

Dosing

Tirzepatide dosing schedule

Tirzepatide starts at a low dose and increases every four weeks so your body can adjust. Many people do well on 5 mg, 10 mg or 15 mg as a maintenance dose. We only move up when you are tolerating your current dose and need more effect.

Weekly routine

How to take a tirzepatide injection

You inject tirzepatide under the skin of your stomach, thigh or upper arm once a week, on the same day each week, with or without food. Rotate injection sites.

Missed dose: take it within four days. If more than four days have passed, skip it and go back to your regular dosing schedule. If you take too much, call us or the Poison Control helpline at 1-800-222-1222.

Birth control: tirzepatide can make birth control pills less effective for four weeks after you start and for four weeks after each dose increase. Use a backup or non-oral method during those times, and stop tirzepatide before trying to get pregnant, as directed by your provider.

Head to head

About 20% average weight loss in trials.

Your results depend on your dose, how long you stay on it, and the protein, strength and sleep habits you build along the way.

Safety

Side effects of tirzepatide

Smaller meals, stopping when you feel full, avoiding greasy foods and drinking enough water help with the common digestive side effects, which usually ease as your body adjusts to each tirzepatide injection.

Serious risks

Less common but serious risks include pancreatitis, gallbladder problems, low blood sugar when combined with insulin or a sulfonylurea, kidney problems from dehydration, hypersensitivity reactions, worsening diabetic retinopathy and possible thyroid tumors seen in animal studies. Because tirzepatide slows the stomach, tell anyone giving you anesthesia or deep sedation, since there is an increased risk of pulmonary aspiration. Call your doctor immediately or get immediate medical attention for severe stomach pain, blurred vision, trouble breathing or trouble swallowing, or a lump in your neck that could signal thyroid cancer.

Brand-name or compounded tirzepatide?

Zepbound and Mounjaro are FDA approved. The tirzepatide shortage ended in late 2024, and compounded tirzepatide is not FDA approved, so its quality and dose are not reviewed by the FDA. Tia will explain which options are available to you and what each costs.

Common side effects

  • Nausea
  • Diarrhea
  • Vomiting
  • Constipation
  • Stomach pain and indigestion

Cost

What tirzepatide costs

Cost depends on your insurance coverage for Zepbound for weight loss or sleep apnea, which varies by plan, and on the form and dose. Zepbound comes as a single-dose pen or a single-dose vial, and vials are often the lower-cost self-pay option. Mounjaro is often covered for type 2 diabetes.
